Germline Testing at Foundation Medicine
Ordering germline testing with one of Foundation Medicine’s CGP assays allows you to build a more comprehensive molecular profile for your patients to help guide clinical management. Germline tests can also identify genetic variants linked to hereditary cancers and help you understand future risks for your patients and their families.

The turnaround time for both FoundationOne Germline and FoundationOne Germline More is 2-3 weeks.
Germline tests through Foundation Medicine require the submission of a separate sample for testing. Acceptable sample types for germline testing include peripheral blood (EDTA) OR Saliva Swab. Sample collection materials for germline testing can be requested by contacting Foundation Medicine at 888-988-3639 or client.services@foundationmedicine.com.
Providers can request kits for germline testing by emailing our client services team at client.services@foundationmedicine.com.
Billing for FoundationOne Germline and FoundationOne Germline More will be completed by our reference laboratory, Fulgent Genetics. All billing inquiries surrounding germline testing should be directed to: 1-888-385-4368 OR billing@fulgentgenetics.com.
Billing for FoundationOne Germline and FoundationOne Germline More will be completed by our reference laboratory, Fulgent Genetics. Financial assistance is available for qualifying patients based on financial need. Fulgent’s application for their financial assistance program can be found here.
Medicare, Medicaid, and all commercial insurance plans will be accepted for both FoundationOne Germline and FoundationOne Germline More. Actual coverage for the testing will vary depending on the patient’s insurance plan and their respective coverage policies. Germline testing is covered by the majority of insurance plans for qualified patients (as defined by the plan’s coverage criteria). Coverage policies for germline testing can vary but are largely based on the patient’s personal and family history of cancer. Post-test genetic counseling is available at no-cost to qualifying patients. Qualifying patients include all individuals who are identified to carry a diagnostic pathogenic or likely pathogenic variant through germline testing.
In order to access no-cost genetic counseling, the ordering provider must submit the genetic counseling referral form to fmisupport@fulgentgenetics.com to initiate the process. Once the form is submitted, a company called DNAvisit will reach out directly to the patient to coordinate a genetic counseling session. The ordering (referring) provider will be sent a follow-up consult note once the genetic counseling session has been completed.
